Change Information in Your Medical or Dental Records
The information in your medical and dental records comes directly from your provider’s office. Discuss any inaccurate information with your provider at your next clinic visit. Your health information is reviewed and updated in your medical record during each visit.
How do I request records for a family member
If you are not the patient, we will need you to provide more information about your right to receive someone else’s records.
We may ask for items like a power of attorney or, if the patient is deceased, a death certificate.

Medical and Dental Record Charges
There is no charge to send medical and dental records to a physician, clinic, hospital, or other health care provider.
Receiving a Birth or Death Certificate
If you need a copy of a birth or death certificate, please visit the Washington State Department of Health.
How do I request radiology images?
For x-rays, please complete and sign an Authorization to Release and Disclose Protected Health Information form and fax it to 425-517-8522.
