Dental
Dental Outreach Program
The Dental Outreach Program offers no-cost, preventative dental services directly to students in school districts throughout Puget Sound, including Edmonds, Mukilteo, Everett, Marysville, Orcas Island, Lopez Island and San Juan Island. Providers attend each school twice a year to do dental screenings. The first screening, called a “Smile Check,” aims to detect early signs of cavities, infections, dental abnormalities, or broken teeth. At the second screening, providers apply fluoride and sealants to minimize the risk of cavities. By delivering these dental preventative services within the school setting, barriers such as parental work commitments, accessibility, finances and language are removed, allowing children to access essential dental care.
Medical
The Medical Outreach Program embeds care teams within shelters, housing facilities, and community organizations where people face significant barriers to care, adapting our services to best meet the needs of clients where they are with a focus on trauma-informed and low-barrier services.
Harborview Madison Clinic at CHC
In partnership with UW Medicine (Harborview Medical Center) and the Washington State Department of Health, Community Health Center of Snohomish County (CHC) hosts the Madison Clinic weekly at our Everett-South location to provide comprehensive medical care to HIV/AIDS patients in their community.
Clare’s Place
Operated by Catholic Community Services, Clare’s Place provides 65 units of permanent supportive housing for chronically homeless and vulnerable households in Snohomish County. CHC has a dedicated clinic space in the building and provides medical services to residents, including a focus on medication assisted treatment, chronic condition management, and care coordination.
Cocoon House
Cocoon House provides a variety of services to support young people experiencing homelessness including operating housing, a drop-in center, education and employment services, homelessness prevention and family engagement resources, and community outreach. CHC has a dedicated clinic space in their U-Turn Youth Drop-in Center where we provide medical services to all program participants with a focus on health education, prevention, and care coordination.
Everett Gospel Mission
Everett Gospel Mission provides emergency shelter and support services to homeless men, women, and children. CHC provides weekly medical services at the Women’s and Children’s Shelter with a focus on preventative care such as immunizations as well as chronic care management and care coordination.
Tomorrow’s Hope Child Development Center
Operated by Housing Hope, an Everett-based organization that is focused on breaking the cycle of poverty and homelessness through housing, employment, and services for children, Tomorrow’s Hope Child Development Center is a credentialed childcare center licensed to serve 135 children from four weeks old to 12 years of age. CHC provides weekly medical services to children at the center, focusing on acute health issues, connection to primary medical homes, and system navigation for staff and families.

Regardless of Insurance
Community Health Center of Snohomish County (CHC) provides affordable health services, including medical, dental, behavioral health, substance abuse treatment, and pharmacy services. We welcome patients who are on Washington Apple Health, Medicaid, Medicare, Medicare Advantage, Tricare, select private insurances, and those who are uninsured. For those without insurance, CHC will help patients sign up for Washington Apple Health; if ineligible, we offer a sliding fee discount based on household income and family size.
