Today, we’re celebrating CHC’s patients! Based on our commitment to provide care for all, community health centers serve patients who are low-income, uninsured, with language barriers, experiencing homelessness, and those living in public housing.
Over two-thirds of the patients who receive care at community health centers across the country are members of racial and ethnic minorities, which is one of the reasons these centers are so successful at reducing racial and ethnic health disparities in our country.
As a Patient-Centered Medical and Dental Home our patients are a key part of the health care team. Our goal is to provide everyone the care they need and in the way they deserve. Providing patient-centered care means that we look at the whole person. We will partner with each patient to offer care that is respectful of their values, culture, and preferences.
