Everything you need to know to make an appointment, prepare, and check-in for your visit!

1. Request an Appointment 

To make scheduling easy, we offer three ways to schedule an appointment.

2. Complete Your Pre-Registration in Phreesia

Here’s how it works:
About five days before your scheduled appointment, you will receive a text and email from Phreesia with a link that will allow you to complete all of your necessary forms and scan your insurance cards. You do not need to register for this service, create a username, or remember another password. Tap on the link, follow the prompts, and you’re ready to go!

If you don’t have a mobile device with web service or a computer at home, you can use one of the tablets available in the clinic office. Our clinic staff will be available to help you if you need it.

    Here’s why it’s important:
    In addition to making your check-in easier, this system also allows your provider’s office to communicate directly with you via email or text to inform you of an emergency clinic closure and other important information. You can text the office with any questions/concerns, and they will go right to the clinic staff!

    Here’s what you need to do:
    Click on the link we send you by text or email before your appointment and follow the prompts. You only have to fill out the forms relevant to your next visit. Once you register, the system will remember your contact information, so you only have to enter it once unless you need to change it later.

    3. Bring the Following Items to Your Visit

        • Medical or dental insurance card, Medicaid coupon, and Medicare card
        • Proof of income (pay stub or tax return)
        • Photo identification (ID)
        • A list of all medications

    4. Check-in at the Reception Desk

        • Arrive 15 minutes before your appointment and check-in with the receptionist even if your pre-registration is complete.
        • Late arrivals may need to be rescheduled.
