What Is Patient-Centered Care?

At Community Health Center of Snohomish County (CHC), providing patient-centered care means that we look at the whole you, from your physical needs to your mental health needs. We will partner with you to offer care that is respectful of your values, culture, and preferences.

Who Is Your Care Team?

An important part of patient-centered care is understanding who makes up your care team. Your main Primary Care Provider is often referred to as a PCP. A PCP can be a doctor, nurse practitioner, or physician assistant. This person is the primary person that you work with on your care team to make decisions about your medical and dental care.

Your Medical or Dental Home is the clinic where your PCP or dentist and a team of healthcare professionals work with you to build a relationship based on your healthcare needs. Your care team might include nurses, medical and dental assistants, behavioral health specialists, social workers, nutritionists, physical therapists, psychiatrists, pharmacists, pharmacy assistants, or other CHC employees. Our goal is to provide you with the care you need in the way you deserve.

Taking responsibility for and being involved in your care is one of the most important things you can do for your health!
